The Yellowknife Health and Social Services Authority (YHSSA) is requesting proposals from qualified proponents to provide; • Group Supported Living Placements for a minimum of eight (8) client spaces. These client spaces require 24 hour supervision. This program is an integrated, flexible support system for all persons with mental health issues that are responsive to the unique needs, interests, abilities and safety of these clients referred through an Adult Assessment and Placement Committee. • Supported Independent Living to a minimum of 20 client spaces. These client spaces require regular home visitation and related supports, depending on client individualized needs. This program is not envisioned to be the client’s sole provider of support. Rather, it will play a core role in facilitating the client group’s connection with other supports and support systems in the community to the greatest extent possible.
